

GREEN CARD LOTTERY
DIVERSITY IMMIGRANT VISA PROGRAM
(DV-2007)
To register by mail to this year’s Green Card Lottery through my office, follow the 5 steps below.
Step 1: Click on the first button at the bottom of the page to print out 2 copies of the Entry form.
Complete and sign the first copy of the form using your usual signature in your native language.
Sign the second copy.
If there are any corrections or changes that need to be made after I review your material, I can make them and complete the second form without having to mail it to you to be signed again, thus saving time.
Please note that your lottery entry form must be signed by you, the applicant, using your usual signature in your native language; no one else can sign it on your behalf.
Step 2: Click on the second button at the bottom of the page to print out a copy of the Contact Information and Client Agreement form.
Complete and sign the form.
The form allows you to choose what mail/contact address you would like listed on your entry form. I offer the possibility of using my office’s address as the contact address for your lottery entry form. You may prefer this option if you plan to move in the near future or if mail service in your country is unreliable. By using my office’s address, the Department of State will contact me directly if your entry form is selected. I then will immediately contact you (by mail, telephone, fax, and/or e-mail) to inform you of the next steps to take to continue with the visa application process. If you prefer, however, to use your own mailing address, you can indicate so on the Contact Information and Client Agreement Form.
Step 3: Include a recent id photograph for yourself and, if applicable, your spouse and your children.
You must provide good quality photographs.
The photograph(s) should be less than 6 months old.
It should be 50 mm square (50 mm x 50 mm) in size.
Do not attach the photograph(s) to your entry form.
Step 4 : Include payment for my services.
Rates for this year’s lottery
(the DV-2006) :
The fee for preparing and submitting 1 lottery entry form is $45.
The fee for preparing and submitting 2 lottery entry forms (for a husband and wife) is $80.
There will be a $30 surcharge for all lottery entry forms prepared in the final 2 weeks before the official deadline.
Please note that payments must be made in U.S. dollars and should be made payable to Edward Weiner, Attorney at Law.

Money transfers through Western Union, Thomas Cook, American Express, etc., which are available through banks and post offices around the world. My office can assist you in finding the closest location in your country.
Checks drawn on a U.S. bank.
U.S. money order.
Travelers cheques.
International money orders.
Foreign postal money orders available through post offices.
